雷公日:于此有人,头痛,筋挛骨重,怯然少气,哕噫腹满,时惊,不嗜卧,此何藏之发也?脉浮而弦[《针灸资生经》第六《头痛》引作“其脉举之则弦”],切之石坚,不知其解,复问所以三藏者,以知其比类也。帝日:夫从容之谓之也。夫年长则求之于府,年少则求之于经,年壮则求之于藏,今子所言皆失。八风菀熟,五藏消烁,传邪相受。夫浮而弦者,是肾不足也,沉而石者,是肾气内著也,怯然少气者,是水道不行,形气消索也,咳嗽烦冤者,是肾气之逆也,一人之气,病在一藏也,若言三藏俱行,不在法也。
Lei Gong said: "Here is a patient who has headache, convulsion of muscle and tendon, heaviness of the bone joints, being timid, short of breath, has abdominal distention, being terrified often, reluctant to sleep, to which viscus does this disease belong? His pulse condition is wiry when holding up, and is hard like stone when being pressed, and I do not know the reason. Now I ask again the conditions of the above three viscera, because I want to know the way of analogy."
Yellow Emperor said: "In analogizing, one should diagnose in a leisure way. Generally speaking, to the elder people, as they often are addicted in delicious food, their disease should be seeked from the six bowels; to the young people, as they are often overstrained in physical labor, their diseases should be seeked from the channels; to the people in the prime of life, as they often indulge in sexual life, their diseases should be seeked from the five viscera. Now, you only state the pulses of the three viscera, and neglect to seek its source, so, you are wrong. The accumulated evil from the eight winds can form heat and consume the five viscera. Besides, the variation of the evil energy can be passed to one another in regular order. The floating and wiry pulse condition you have said shows the insuffciency of the kidney energy; the pulse is as hard as stone instead of sunken when being pressed heavily shows the kidney energy is stagnated inside without moving; in this case, the turbid Yin will not descend, the body fluid will not be spread to cause dissipation to the physique and vital energy, and the up-reversing of the kidney energy causes cough and disquietude. Therefore, the disease should be in the kidney. If you hold that the disease is in all the three viscera of liver, spleen and kidney, it is not tally with the medical classics."
雷公日:于此有人,四支解堕,喘咳血泄,而愚诊之,以为伤肺,切脉浮大而紧,愚不敢治,粗工下砭石,病愈多出血,血止身轻,此何物也?帝日:子所能治,知亦众多,与此病失矣。臀以鸿飞,亦冲于天,夫圣人之治病,循法守度,援物比类,化之冥冥,循上及下,何必守经。今夫脉浮大虚者,是脾气之外绝,去胃外归阳明也,夫二火不胜三水,是以脉乱而无常也。四支解重,此脾精之不行也。喘咳者,是水气并阳明也。血泄者,脉急血无所行也,若夫以为伤肺者,由失以狂也,不引比类,是知不明也。夫伤肺者,脾气不守,胃气不清,经气不为使,真藏坏决,经脉傍绝,五藏漏泄,不衄则呕,此二者不相类也。臂如天之无形,地之无理,白与黑相去远矣。是失,吾过矣。以子知之,故不告子,明引比类、从容,是以名曰诊轻,是谓至道也。
Lei Gong said: "Here is a patient who has slothful and weak extremities, rapid breathing, cough and hematochezia. I diagnosed and thought the disease was caused by the lung injury, but his pulse condition was floating, gigantic and empty, so I dare not treat him. A physician of lower level treated him with stone needle and the bleeding was plenty. But after the bleeding, the patient felt relaxed in the whole body. What is the disease?" Yellow Emperor said:"Although you have known and cured many diseases, yet you are wrong regarding this one. The phycician of a lower level can cure the discase occasionally like the aquatic bird can fly into the sky occasionally.
"The superior physician adheres to the standing orders, reasons the disease by analogy, analyses it through pondering and treating according to the changing situation. When he inspects the upper part of the body, the lower part can also be known, and he may not adhere to the condition of channel rigidly.
"When the pulse condition of the patient is floating, gigantic and empty, it shows the spleen energy is pouring into the stomach, and the body fluid joins the Yang solely.
"When the second fire (stomach) is incapable to control the third water (the third Yin or the spleen), the pulse condition will become disorderly.
"The slothful and weak extremities show the inability of spreading the essence of the spleen, the rapid breathing and cough shows the combination of the water energy to the Yangming channel, the hematochezia shows the shrinking of the channel which obstructs the blood circulation to cause the blood overflowing outside. If you think it is due to the lung injury, it is an error of wantonness. The reason of which you can not reason by analogy is because you do not have a clear comprehension about it.
"If the disease is caused by the lung injury, the spleen energy will not be maintained,the stomach energy will not be clear, the lung energy will be damaged and deteriorated and the refined energy will not be able to be spread by the channels. As the refined energies of the five viscera are leaking, the diseases of hemorrhage or hematemesis will occur.
"There is a large difference between the disease of lung injury, and that of the spleen injury. They are different like the heaven and earth, and like black and white.
"Your failure in diagnosis is also my fault, I thought you have understood it well already, and I did not remind you the standing orders of the analogy and the diagnosis in a leisure way which are surely the brilliant theory and the quintessence in diagnosis."
